The pilot of the U.S. Army Black Hawk helicopter that crashed into an American Airlines plane in DC made a fatal mistake in the moments before the collision, according to a new report. All 64 people on board the plane and three crew members on the Hawk were killed during the mid-air collision in Washington DC on January 29.
